Some horses have EIPH due to genetics of the respiratory system.  Many horses develop EIPH due to conditions as how heavy their hooves hit the ground during competition or a turf horse who should be running on dirt or a dirt horse who should be running on turf or how the horse is shod.  A horse that hits the ground heavy can cause a ripple in the chest cavity, which causes or contributes to EIPH.
